Information on the processing of patients' personal data.

Your personal data is processed in relation with the provision of healthcare services. The administrator is MUDr. Tomáš Doležal Plastika Praha, Národní třída 32, 110 00 Praha 1, IČO: 62887467 (“the Administrator”)

The purpose of processing personal data is primarily to provide healthcare services (processing is necessary to fulfill the legal obligations of the Administrator) within the scope of the following data: first name; surname; address; date of birth; gender; personal identification number; insurance number; information on legal capacity; profession; information about health status and the procedure, course, and results of the provided healthcare services; family, personal, occupational, or social history. Your data will be processed for this purpose for the necessary period, as stipulated by Annex No. 3 of Decree No. 98/2012 Coll., on medical documentation. In the case of accessing medical records, your identity may also be verified by processing your identification document number and your signature. Please be informed that without providing the above-mentioned personal data, healthcare services cannot be provided to you.

In connection with the provision of healthcare services, personal data is processed to a limited extent outside of medical records, for example, in scheduling systems for the purpose of planning your appointments (name and surname, contact details – phone, email, delivery address, type of healthcare service) or in documents sent to health insurance companies (insurance number and provided healthcare service) and other accounting records if the services are not covered by public health insurance (name, surname, address, and provided healthcare service). These data are also processed due to legal obligations or the Administrator’s legitimate interest, which lies in work planning or demonstrating compliance with legal obligations. The data is processed only for the necessary period: in the case of scheduling systems, for one year; in the case of documents for health insurance companies, for the period specified in the contract with the respective health insurance company; and in the case of other accounting records, for the period prescribed by regulations governing the keeping of tax records and accounting.

Personal data within the aforementioned scope is processed on the basis of legally established grounds, and therefore without the need for your consent.

The Administrator will process personal data in accordance with the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC ("GDPR").

Personal data will be shared with third parties, specifically other healthcare service providers; health insurance companies; the Administrator's processors; and relevant public authorities in accordance with applicable legal regulations. Personal data will not be transferred to other countries.

During the processing of your personal data, you have the right to:

  • a) Request information from the Administrator regarding whether your data is being processed, for what purpose, which personal data is being processed, the sources of this data, the planned duration of processing, and the recipients of the personal data. If you request information about the processing of your personal data, the Controller will provide you, without undue delay and free of charge, with a copy of the processed data (the Controller may charge a reasonable fee for any additional copies);
  • b) Request the correction of inaccurate personal data or the completion of incomplete personal data concerning you;
  • c) File a complaint with the Office for Personal Data Protection located at Pplk. Sochora 27, 170 00 Praha 7 ([javascript protected email]);
  • d) Request the deletion of personal data and the right to object the processing; however, these rights do not apply to personal data that the Administrator is required to process to fulfil legal obligations, such as in connection with maintaining medical records.
